Register with our practice

We welcome new patients who live within our practice area to register with us.

The Practice catchment area covers Southam and the following villages: Bascote, Long Itchington, Stockton, Napton on the Hill and Ladbroke.

Anyone in England can register with a GP surgery. It’s free to register. You do not need proof of address or immigration status, ID or an NHS number.

GP surgeries are usually the first contact if you have a health problem. They can treat many conditions and give health advice. They can also refer you to other NHS services. You do not need proof of ID to register with a GP, but it might help if you have one or more of the following:

Passport | Birth certificate | HC2 certificate | Rough sleepers’ identity badge | Hostel or Accommodation registration |Mail forwarding letter

If you’re homeless, you can give a temporary address, such as a friend’s address, a day centre or the GP surgery address.

Please note: You will be asked to upload the ID proof in case you request online access to medical records.

What are the acceptable examples of Photo ID and Proof of Address?

Acceptable photo ID:

  • Current passport or identity card
  • Resident permit issued by Home office
  • Current UK photocard driving licence
  • HM Forces identity card
  • Police photographic ID card
  • University or college photo smartcard
  • NHS smart card

Acceptable proof of address:

  • Recent utility bill (within 3 months of the current date)
  • Local authority tax bill ( valid for current year)
  • Bank or building society statement ( within 3 months of a current date)
  • Current tenancy agreement
  • Court order (within 12 months of the current date)
  • UCAS letter (within 12 months of the current date)

How do I register my baby?

You need to register your new baby at the Practice as soon as possible.

Parents or guardians can register a baby at the Practice by completing and presenting to Reception, form FP58 which is issued at the same time as a birth certificate. This is for new born babies only. Older babies and children should register using our Join the Practice Request online form.

Mother and Baby Checks

Once your baby is registered, the Practice will send you an appointment for a mother and baby post-natal check. This takes place at around 6 weeks.

This is a 20 minute appointment; 10 minutes for the baby’s check and 10 minutes for the mother’s post-natal check. Further details about what happens at this appointment is available from NHS choices.

Please remember to bring the red book to your appointment.

If you cannot attend this appointment you must notify reception as soon as possible.

Catchment Area (Practice Boundary)

Named GP

All registered patients have a named doctor who has overall responsibility for their care and support. Your registered GP is also your named accountable GP.

Your allocated GP will be responsible for the provision of your healthcare. However, if you choose to see another doctor at the surgery you are entirely free to go on doing so.

If you are unsure of who is your named Doctor please ask at reception.

Patients have the right to express a preference of doctor on their records. However, the practice may not always be able to comply with your request. If this is the case, we will explain why.

Access to Medical Records

In accordance with the Data Protection Act 2018, General Data Protection Regulations and Access to Health Records Act, patients may request to see their medical records. There is no charge for this as long as the request for information is reasonable.

Please speak to administration team if you require a copy of your medical records.

No information will be released without written patient consent unless we are legally obliged to do so.
