We can confirm that invitations to receive the COVID-19 vaccination have now started and local vaccinations are commencing this week. Invitations will initially be made via telephone and text message. Invitations will be sent gradually according to both the supply of vaccine from the NHS, and according to the priority groups set out by the Joint Committee on Vaccination & Immunisation. Midwife/Antenatal Self Referral
Congratulations on your pregnancy. You don’t need to see the GP when you get pregnant, although we are always happy to see you.
Please fill the form to register for your initial Midwife appointment.
Once this form is submitted, the midwifery team will contact you within 5 working days to arrange a booking appointment for when you are between 8 and 10 weeks pregnant. At this appointment you will be given further information about the antenatal care you will receive during pregnancy. You will also be offered an ultrasound scan at around 12 weeks to ascertain when your baby is due.
If you have difficulties answering the questions on the form, any concerns, or have not heard from the midwifery team within 5 working days, then please contact the Bluebell Birth Centre (the midwifery base at Warwick Hospital) on 01926 495321 ext 6977 and they will assist you. The midwifery team can also be contacted via email at AmethystMidwiferyPractice@swft.nhs.uk
