Register with our practice
We welcome new patients and accept registrations from patients living both within and outside our practice boundary.
Our practice catchment area covers Southam and the surrounding villages of Bascote, Long Itchington, Stockton, Napton-on-the-Hill and Ladbroke.
Patients who live outside our practice boundary may register as an out of area patient, subject to practice approval. Please note that the practice cannot provide home visits outside the practice area.
Out of area registration is not suitable for patients who are:
- Housebound
- Pregnant
- Receiving palliative or end of life care
- Undergoing active cancer treatment
- Requiring ongoing or complex mental health support
- Paediatric patients (under 18 years of age)
If your circumstances change and you fall into one of the above categories, we may advise you to register with a GP practice closer to your home to ensure you can access appropriate local services and home visiting support when required.
Anyone in England can register with a GP practice. Registration is free, and you do not need proof of address, immigration status, identification or an NHS number to register.
GP practices are usually the first point of contact for healthcare needs. We can diagnose and treat a wide range of conditions, provide health advice, and refer patients to other NHS services when appropriate.
If you are homeless, you can still register with a GP. You may use a temporary address, such as a friend’s address, a day centre, hostel, or, where appropriate, the practice address.

Named GP
All registered patients have a named doctor who has overall responsibility for their care and support. Your registered GP is also your named accountable GP.
Your allocated GP will be responsible for the provision of your healthcare. However, if you choose to see another doctor at the surgery you are entirely free to go on doing so.
If you are unsure of who is your named Doctor please ask at reception.
Patients have the right to express a preference of doctor on their records. However, the practice may not always be able to comply with your request. If this is the case, we will explain why.
Access to Medical Records
In accordance with the Data Protection Act 2018, General Data Protection Regulations and Access to Health Records Act, patients may request to see their medical records. There is no charge for this as long as the request for information is reasonable.
Please speak to administration team if you require a copy of your medical records.
No information will be released without written patient consent unless we are legally obliged to do so.
