Flu Clinic

Flu vaccinations are available for patients who are 65 years of age and over AND those with the following conditions- Asthma Diabetes, Heart Renal or Chronic respiratory Disease, are Immunosuppressed have MS or are Pregnant.

Travel Advice


Our nursing staff offer travel advice to patients.

A pack has been provided for patients wishing to book a consultation and we ask, wherever possible, that this is collected from surgery before your appointment. The important information within the pack will help you and your family stay healthy whilst travelling abroad.

Please read the pack before your appointment with the nurse and make a note of any particular questions you wish to ask.

The nurse will inform you of recommended vaccinations for your chosen destination.

A charge is made for some vaccinations, as the surgery has to purchase the vaccinations for patients who wish to travel.

Please not that the surgery has to charge for this non NHS service.

Unfortunately due to the amount of patients failing to attend booked travel appointments, it has been necessary to introduce a small charge for non attenders. The cost is £10 per person per single appointment. If you fail to attend for your booked appointment and do not let the surgery know you will be invoiced as above.

Patient Participation Group (PPG) ...


PPG's make a real difference, constructive changes to the provision of services at general practice level.
Helps with aiding the responsiveness of the practice and its ability to provide a service which patients want and need.

All patients are welcome to attend these meetings we appreciate the support.

The Southam Surgery PPG meets quarterly with representatives from the surgery.


The surgery values ideas and comments from the group.  Surgery issues previously discussed include waiting room seating, telephone access, the introduction of the new Doctor contract. Our meetings are informal.  We invite speakers for the first half an hour, followed by a brief Agenda.

Previous speakers have included the local Pharmacist, a dietician, chiropodist and even a beautician – where one of the doctors benefited from a facial! Members of the group provide tea and coffee at surgery events such as the flu mornings.  They are presently raising money to purchase some new equipment for the childrens area.

Summary Care Records ...


All patients 16 years and above will receive an information pack from Warwickshire PCT with information regarding Summary Care Records (SCR).


Patients will have the option to "OPT OUT" of the programme, to do this they will have to complete an "OPT OUT FORM", which they can order using the order form in their packs, or by calling 01926 815842 and requesting one, or by using the website www.nhsrecords.nhs.uk.


The information will only be used in unscheduled care settings i.e. Out of Hours, A&E, wall in centres etc.


The information will only contain, medication, allergies, adverse reactions and discontinued medication up to 6 months.


The information can only be accessed by authorised staff using a smartcard.


If new patients register at the practice and have come from outside of Warwickshire they will need to be given a pack for each new patient over 16 years.  If the patient is moving within Warwickshire they will already have received the information.


Children under 16 years will not receive letters, if a parent or guardian wishes to opt them out they must complete an opt out form for each child.


If a patient has already opted out, they do not need to complete another form.


When a patient arrives for treatment at the unscheduled care setting they will be asked for permission to access their information, if the patient is not happy they can say no at that stage.  This would only be ignored if the patient was unable to communicate, or if it was a court protection matter.


 

Freedom of Information


This Publication Scheme is a complete guide to the information routinely made available to the public by Drs Alastair Robson, Michael Wright, Lesley Adams and Claire Stevenson. It is a description of the information about our General Practitioners and Practice, which we make publicly available. It will be reviewed at regular intervals and we will monitor its effectiveness.

How much does it cost?
The publications are all free unless otherwise indicated within each Class[with £]. Where information is provided at a cost the charges will be calculated as set out Class 7.

Your rights to information

  • In addition to accessing the information identified in the Publication Scheme, you are entitled to request information about Southam Surgery under the NHS Openness Code 1995.
  • The Freedom of Information Act 2000 recognises that members of the public have the right to know how public services are organised and run, how much they cost and how the decisions are made.
  • From 1st January 2005 it will oblige the General Practice to respond to requests about information that it holds, and is recorded in any format and it will create a right of access to that information. These rights are subject to some exemptions, which have to be taken into consideration before deciding what information it can release.
  • New environmental information regulations may be introduced as early as 2003. These will enable similar access to environmental information as under the Freedom of Information Act 2000.
  • Under the Data Protection Act 1998, you are also entitled to access your clinical records or any other personal information held about you and you can contact the Practice Manager at Southam Surgery, Stowe drive, Southam, CV47 1NY to do this.

GP net earnings 2014-15 ...


All GP practices are required to declare the mean earnings (average pay) for GPs working to deliver NHS Services to patients at each practice.


The average pay for GPs working at Southam Surgery in the last financial year was £50,101 before tax and national insurance. This is for 1 full-time GP and 4 part-time GPs.
